For every AIHA qualifier that a youth, adult or women’s team intends to participate in, the team must submit a completed TEAM ROSTER & ENTRY FORM to the AIHA. This form must be completed and accompany the team’s $95.00 qualifier deposit by no later than the designated registration deadline. The TEAM ROSTER & ENTRY FORM must include every player that is officially rostered on the team, along with their official jersey number. Just as important, teams must indicate which player(s) they wish to officially delete from their team roster.

On the middle portion of the form, roster additions and deletions (roster changes) must be entered. If a player or players are being added to a roster after a team has already completed a qualifier, then that player or players must be entered on the form and a $15.00 fee must be submitted to the AIHA for every player being added to the team roster. There is no fee to remove a player from a roster.
